The mail option is for providers who cannot use or choose not to use online claiming. If you are a DVA health care provider, you can mail your claim to us after you consult with or treat DVA clients.
Follow the steps to claim
There are steps you must follow to mail a claim to us.
There are time limits on claims.
Check the latest fee schedules before you claim.
Step 1: find the right claim form or voucher
- D0652B — Hospital Admission voucher
- D0653A — Discharge Advice & Hospital Claim
- D0688 — Medical Grade Footwear (MGF) Prescription
- D0800 — Claim for travelling expenses
- D0904 — DVA Request/ Referral voucher
- D0931 — Spectacles Prescription
- D0986 — Dental Request
- D1083 — Community Nursing Service voucher
- D1217 — Claim for Treatment Services voucher
- D1328 — Prior Financial Approval Request

Step 2: fill it in and print it
We prefer that you use your computer to fill in the form or voucher.
This means you can save the form or voucher for your records before you print it out.
Step 3: mail it to us
